Eligibility criteria:

• - Be a business or not-for-profit entity.
• - Be headquartered in Jefferson, Bullitt, Oldham, or Shelby counties (Kentucky), or Clark, Floyd, Harrison or Washington counties (Indiana).
  Employ a minimum of 5 full time employees, including the owner.
• - Demonstrate evidence of an employee wellness program.
• - Complete the online application.
